botox – how many patients have it?

I would say around 90% of my patients that come and see me undertake botox procedures for the upper third of the face. Whether this be for 1 area (around the eyes), or 2 areas (forehead and frown) or all 3 areas.

 

Another 5% see me for botox in the lower face (smokers lines, chin or to lift the corners of the mouth) or under the arms.

 

Then the remaining 5% will see me for dermal fillers. Out of the patients that have dermal fillers, half are for lips (whether to redefine them or make them fuller) and the remaining half are for the nose to mouth lines.

 

Botox on average would last for 3-6 months and dermal fillers you would be looking in the region of 9 months. To achieve optimum results we would not suggest botox and dermal fillers as being one off treatments, the more you have these procedures, the better the final results are going to be.

 

There are a  minority of patients that unfortunately we cannot help with botox or dermal fillers. These include patients with loose skin either generally or under the eyes. They would normally require surgery to sort out their concerns. In addition anyone with scars would be hard to treat with the treatments we offer. We do offer certain skin care range that can improve the appearance of scars but would not eliminate them totally.

 

We always ask patients to take advantage of our free consultation if they are unsure on which treatment is suitable for them. Once we see them face to face we are in a much better position to explain what can be achieved and what options are available to them.

I met the author in 2008 when he won an Award for ‘Best Marketing’ initiative at the Private Practice Awards in 200X. At that stage I had a more active interest in Facial Aesthetics so a conversation with him was fruitful and fun. My own training in Facial Aesthetics very cleared showed that although it could appear that dentistry was intruding into medicine, these techniques are definitely more effectively delivered by dentists in their practices. Botox and Lord Sugar

A lot of press, mainly bad press, has been written about Lord Sugar and his decision to go into partnership with the winner of The Apprentice, Dr Leah who plans to open a chain of botox clinincs.

 

Initially, I had nothing agains this. All exposure is good and if the TV series has gotten more people interested in the effects of facial aesthetics and notably botox, then I am all for it. It may also, a big may though, speed up the regulation process, so only properly qualified medical professionals, such as dentist, will be allowed to carry out these non surgical procedures.

 

However, as I looked a bit more deeply into her credentials and plans, then alarm bells began to ring.

 

Dr Leah is only newly qualified. My first gripe, is that she wants to leave the NHS (who have paid to some extent for her several years of education) to enter the private sector. As a newly qualified graduate I cannot understand how she has developed the necessary clinical excellence and experience needed to become a facial aesthetician. I have been carrying out facial aesthetic treatments, such as botox since 2002 and I am still continually updating my skills and learning new material.

 

The other concern, is that she is driven, it appears by the financial aspect of facial aesthetics. This is contrary to the oath that we dental/medical professionals agree to. Financial issues do not come into play whatsoever when discussing treatments and recommendations with patients. Lord Sugar is a business man and will want to see profits form his investment. How will she manage on one hand the ethical standards she has been taught against the riches Lord Sugar will demand. Only time will tell.

botox – what happens at each visit?

We would carry out a consultation first. In this visit we would answer any queries you may have. Then we would carry out a detailed assessment of the areas that you are concerned with. We would then show you some before and after pictures of similar cases to yours that have been treated with botox. Then we would present you with a detailed treatment plan on all the possible options available to you.

 

We would then give you time to think about all the options and have any further questions answered. Once you are completely happy with your choice of treatment, such as botox, we would go into more detail about the procedure and special precautions you would need to take after the procedure.

 

If you have opted for botox treatment, this would normally take 2 visits. The first visit which would last 30 minutes is where the procedure is carried out. In addition we would go over the consent procedure and take before photos.

 

After 2 weeks you would come in for the review appointment. This is where we may need to give you a top up of botox on any areas that have been resistant to the original dose. We would also take after photos.

 

We would next see you when you are due some more botox which should be around 3-6 months.

botox – how to guarantee the results

With any medical procedure, due to the fact we are working with the human body, it is impossible to guarantee any procedure. However saying that we can with experience and looking back at our own results be very confident of hat results we can achieve by using botox.

 

It is very rare – in my 10 years experience of administering botox, I have only had 2 cases – that someone may not react to the botox treatment. So you can be reassured that botox will achieve the results you wish for.

 

We also guarantee all our botox treatments for 3 months. So after the initial treatment, if you feel you need some more or the lines have reappeared, you can come back as many times as you want within a 3 month period and obtain free top ups.

 

There are certain patients that we will not treat with botox, such as pregnant or breast feeding women, patients allergic to eggs or patients with certain neuro muscular problems. It is not that botox wont work in these patients, it is that they may have potential side effects that we would wish to avoid.

 

In summary, if you are not contra indicated to have botox then you can almost guarantee that botox will work for you and you will also have the reassurance that we guarantee the results for 3 months.

botox – which lines can be treated

Many patients are unsure what areas of the face and which lines or wrinkles botox can treat.

 

Generally botox can be used for any lines that are caused by you using that particular muscles. It cannot be used for deep wrinkles or folds that have not been caused by the muscles working.

 

Now that we know which type of lines and wrinkles are suitable for botox treatment, lets talk at different areas of the face that could benefit from botox.

 

Normally botox is used for the upper third of the face, this is true in 90% of the cases I treat, but it can be used for other muscles in your face.

The commonest areas botox is used are:

 

  • frown – the muscle between your eyebrows, when this muscle contracts you get the vertical lines, also known as hammer lines or number 11’s
  • Forehead – when you contract your forehead muscles you get the horizontal lines across your forehead
  • Eyes – when you smile you get the lines at the side of the eyes, also know as crows feet. Please bear in mind that we don’t use botox directly under the eyes.

 

These are the commonest 3 areas that botox is used to treat, however we can still use botox for other smaller muscles in the lower face area, such as:

 

  • smokers lines, these are small vertical lines either in the upper or lower lip region
  • Corners of the mouth – you can use botox to lift the corners of the mouth to reduce or eliminate a ‘sad face’
  • Chin – also known as the ‘orange peel effect’
  • Jaw line – you can tighten the angle of the jaw
  • Massetter muscles – your chewing muscles can be treated to reduced the squareness of your face
  • Neck – we can use botox to reduce the lines across your neck

 

So now you know what areas of the face can be treated with botox.

botox alternative – fillers, different types

Many patients ask me what types of fillers are available to them and what make would I recommend.

 

I personally do not recommend one particular brand. Each company/brand have several fillers in their range of products and I choose which one is best for my patients demands. So I might use one filer from one company and another type of filler from another company.

 

Broadly speaking fillers can fall into 3 sub categories. They are divided into particular size, the larger the particular the longer the product will last.

 

Fine fillers – these have the smallest particle size and have the shortest duration time. These are used for very superficial lines, such as smokers lines that can occur above your upper lip. You don’t want a heavy particle fillers, since the skin is so thin in these areas, a heavier filler will cause lumpiness which may be visible. The effects of these fine fillers should last up to 6 months.

 

Medium fillers – as the name suggests these have average sized particulars. These type of fillers are normally used for the lips, nose to mouth lines and moth to chin lines. They will plump up the skin and are injected slightly deeper into the skin. The effects should last around 9-12 months.

 

Volumerisers – these are the heaviest particle sized fillers. As the name suggest they will add volume to the area treated. Common areas for these type of fillers are the cheeks. They are injected deeper than the other 2 fillers and the effects can last from 18-24 months.

 

We can control the look you want by using different graded fillers to achieve your desired look.

This is one of the most frequently asked questions we get asked when anew patient is enquiring about botox treatment.

 

The biggest reaction we get is that they thought it would be a lot more expensive.

 

Botox is charged by the area you have treated.

 

In our clinic we have the following prices.

 

1 area which is the eyes is £140

 

2 areas which are the forehead and frown are £190

 

3 areas which are the eyes, forehead and frown are £235

 

Extra areas, such as upper lip, corners of mouth, bunny lines (lines on side if nose when you scrunch it) are £99 independently or they are reduced to £65 if combined with other areas on the face.

 

We offer excellent value for money. The reason we can make our prices competitive is that we do so many botox procedures, that we need to order large quantities of botox and therefore manage to negotiate a discount on the stock and we can pass this saving onto you, the patient.

 

So hopefully cost should not be a barrier to allow you to reduce or eliminate those unwanted lines and wrinkles.

Dermal fillers -what to do after the procedure

Dr Singh who runs his botox clinic in Stevenage, Hertfordshire explains what you need to do after your dermal fillers procedure.

Following your treatment it is important not to touch the treated area or apply makeup for at least 6 hours. This is to reduce the risk of injection site infection. Please bear in mind that immediately after treatment some common anticipated injection related reactions can occur. These reactions are temporary and typically resolve within 7-14 days. However in order to allow the skin to recover more efficiently the following advice may be proven useful:

 

Redness or Swelling

 

A clean, light make-up can be applied to cover any redness after the initial 6 hour recovery period.

Do not expose the treated area to intense heat (sunbathing, sun beds, saunas, steam rooms etc) or to extreme cold 1 week following treatment. A clean cool pack can be applied directly to the treated area on the day of treatment to help with any bruising.

 

Bruising

If you are using Aspirin or similar medication be aware that the likelihood of bruising may increase.

It has been reported that Arnica* cream and/or tablets help to minimise bruising. Arnica cream can be applied directly to bruising and may help the bruise to resolve more quickly. Make-up can be applied to cover bruising following the initial 6 hour post treatment period.

 

Discomfort or Itching

 

Following treatment the area can feel tender from the injection. Non-prescription analgesia* can be taken. Following treatment the area may feel slightly itchy for 7-14 days. Non prescription anti-histamines* may reduce itching.

 

Lip Treatment

Due to sensitivity of the lip swelling following treatment can sometimes last for up to 7-14 days and during this time the lips may appear to be slightly uneven. The result directly following treatment should not be seen as the final result.

A clean cool pack can be applied directly to the area to soothe the lips and help to reduce swelling.

 

If you have a history of cold sores, which you should advise DR Singh, there is a risk due to the insertion of a needle into the skin where the virus is present, but dormant that the virus may be activated. This may cause a first outbreak of a cold sore or another eruption from a previous one.

 

Restylane Vital

Following treatment with Restylane Vital the aforementioned anticipated side effects may occur. A small number of patients have also reported experiencing raised nodules at the treatment site some hours following treatment. This is a normal reaction to the treatment and typically resolves within 5 days, however, on rare occasions may persist for 7-14 days.
