Botox – pre requirments

Dr Singh who runs his botox clinic in Stevenage, Hertfordshire, in his latest blog illustrates that there are many systems and space requirements needed before any practitioner can offer botox injections in a safe environment, and of course not forgetting adequate training.

Some of these are included below and is part of the information I give to my professional colleagues interested in offering botox treatments.

Insurance requirements

Your normal dental/medical indemnity will not cover you fro any facial aesthetic procedures.

At the time of writing these mini tips there 2 providers of indemnity for facial aesthetics:

 

  1. Hamilton Fraser – you will need to list all the procedures you wish to undertake, which products you have been trained in and show the necessary certificates to confirm training
  2. MPS – this can be added onto your existing dental indemnity but is restricted to the neck upwards. So you will not be covered for underarms and any hand treatments

 

Please check for latest requirements from each of these indemnity providers.

 

Space requirements 

 

In an ideal situation, i would recommend the following:

 

  • separate consultation room – so the patient can speak in privacy regarding their concerns. You would also carry out the consultation and assessment here. So you need good lightning, a large wall mirror, PC  to show them the pre treatment photographs
  • separate clinical area – this would then differentiate it from your dental/medical treatment room. You want to give the ambience of a spa room, so the patient is relaxed. All you would need int eh room is a couch and display of your skin products

 

I can appreciate because of space and budget constraints that this might not be possible. You can always modify the above ideal scenario to your particular situation.

 

Team training 

 

You cannot do this alone. You need the backing and support from all your team members.

 

When you go on any clinical courses on facial aesthetics update your team members on what you have learnt. Discuss with them your plans for the facial aesthetics business and get their input. Outline your marketing plan and delegate some of the roles to them – they will like the responsibility.

 

Reps are excellent in explaining in plain english regarding the different procedures to your team members.

 

Teat your team members for free – they will be a walking advert. i have lost count on the number of times a patient will ask a team ember if they have had any procedures, and since they have, they automatically convert that patient and gives them peace of mind.

 

 

Treatment menu 

 

Many clinicians when they stat off make the mistake of offering everything at the beginning and confusing themselves, their team and the patient.

 

In my situation, I started in the following order:

  1. botox injections and skin range
  2. Dermal Fillers
  3. Volumerisers
  4. Advanced Toxin use – underarms, migraines

 

I would become a master in the first tier before moving onto the next tier. You will be more confident and happy providing the procedure and your team will become experts at a particular procedure before moving onto the next one.

 

Once you have established yourself and offer all the above procedures then you can promote this internally via your medical history questionnaires where you have a tick box to see if the patient is interested in any of the services.

1.  free post kit after the botox procedure – these can easily be obtained from the manufacturers. It cost us nothing and is given to every patient after any procedure they have. They love getting stuff for free and you can also cross sell your skin range at the same time.

2.  free mineral makeup session after the botox treatment – similar to above, these patients are concerned with their appearance, so why not do a mini makeover with your mineral make up. They are not expecting this and you never know, they may want to purchase some of your mineral make up range.

3. free neck and shoulder massage during the botox treatment – Jenna, my assistant, is a qualified beautician. So whilst I am preparing the toxin/filler, she will give the patient a free 2 minute massage.

4.  3 month guarantee – on all our botox patients we offer a 3month guarantee from the day of their treatment in that if they feel they need  atop up or it has worn off within 3 months, it will be free of charge. You will get a tiny minority that may abuse the system, but this is counter balanced by the patients that appreciate this safety net and tell their friends about this.

5.  mates rates – we normally charge £235 for 3 areas of botox. We will reduce this to £200 per person, if they bring 2 friends with them to have it done at the same time. You will be amazed on who our patients will try to encourage to come so they can save £50. If they take this offer then the above referral incentive is not applicable.

This is an excellent combination treatment with botox injections. As we know botox will reduce or eliminate fine lines and wrinkles and the Dermaroller will improve the skin complexion.

 

I understand that the procedure can result in an appearance enhancement and is typically used for skin rejuvenation and scar repair and that the treatment uses a DermarollerTM medical device that creates controlled micro-medical nee- dle punctures of the skin surface. I also understand that I may require a series of DermarollerTM treatments, normally with at least 6 weeks between procedures, to achieve the maximum cosmetic result. I acknowledge that no written or implied verbal guarantee, warranty, or assurance has been made to me regarding the outcome of the procedure.

I understand the following:

  • That immediately after the DermarollerTM procedure the skin will be red, resembling moderate sunburn, as
    the skin naturally heals the redness will resolve. The skin may remain red for three to four days after Der- marollerTM treatment, although it is usual for it to subside within two days and many people are able to return to their normal activities the same or next day. It is recommended that the use of soaps on the treated skin area is restricted until the redness subsides and where possible warm / tepid water is used for cleansing. If you are taking any medication or dietary supplements that can affect platelet function and bleeding time (see Ques. 8 overleaf) the period of redness can be extended.
  • The DermarollerTM procedure can cause areas of bruising although this would not normally be expected to occur, the eye contour being the area at most risk. If you are taking any medication or dietary supplements that can affect platelet function and bleeding time (see Ques. 8 overleaf) the severity and period of bruising can be extended, also the presence of petechiae (small red or purple spots beneath the skin) may be ob- served.
  • There is a small risk of infection of the treated skin area after the DermarollerTM procedure although this is not expected to occur due to the sterility of the Dermaroller® medical device and the minimally invasive na- ture of the micro-medical needles.
  • There is a small risk that hyper-pigmentation of the skin can occur after the procedure, although this is not normally expected. Failure to follow the sun exposure and sun protection advice detailed below can increase this risk.

who does it treat?

 

Botox can be used for any lines that are a result of facial expressions. The three main areas are: forehead, frown and lines around the eyes. It is also commonly used to treat hyperhidrosis (excessive sweating under the arms) and some areas of the lower face, such as the chin, corners of the mouth, upper lip.

 

what problems does it solve?

 

Any lines that you are concerned with. It will improve the appearance of fine lines and wrinkles and give the skin a more glowing appearance. You will still be able to move your face and we do not advocate the ‘frozen look’.

 

how can I get it?

botox – questions during the assessment

The assessment is the time the patient can have all their questions and concerns answered in an unhurried manner and non threatening environment. This is critical for the patient and then they can go home and think about all the options and whether they want to proceed with any treatment and if that particular facial aesthetics practitioner is best suited for their needs.

 

Now lets look at the questions I would typically ask at the initial assessment for botox patients. The questions are open ended, this is so the patient can explain in more detail what their requirements are and what kind of results they would like to achieve. It also stops me putting words into the patients mouth.

 

I normally start off with 4 questions and with these 4 questions I can get a good idea on what treatment I would recommend, whether it be botox or fillers.

 

Here are the 4 questions I would ask:

 

HOW CAN I HELP YOU – SKIN COMPLEXION,LINES/WRINKLES, VOLUME

 

 – WHAT IS IT ABOUT THAT, THAT BOTHERS YOU

 

 – HOW LONG BOTHERED YOU AND WHY NOW

 

 – IF HAD DONE BEFORE – WHY CHANGED

 

 

 

Once I have had the answers the patient wants to give me I then explain certain procedures and use a number of props to better demonstrate what botox or dermal fillers can do and what the typical results may look like.

 

I dont use all the propos all the time, just the best suited for the patients needs and these included:

 

 – PT MIRROR OR PHOTOS – some patients are happy holding a mirror in front of them and pointing areas of concerns. Whilst other patients f not want to point to their lines and feel self conscious. Therefore I find talking a photo for these patients, makes them distance themselves from the face and mirror and they are more open too point to areas of concerns.

 

 – POWERPOINT PRESENTATIONS – I have a number of before ad after photos for various treatments such as botox, dermal fillers, but also in each division, lets say, botox, I have sub divided this into different areas and different kinds of final results that can be achieved. Once I know what areas the patient wants treated with botox and what final result they desire I can choose the correct case to demonstrate this to them.

 

 – USE ALLERGAN 3D POSTER – this is a 3d image of all the muscles in the face. This is useful for anyone considering botox and only wanted a particular area. With this 3d muscle poster I can show how the muscles interact with each other and why it maybe not ideal just to have 1 area.
